Job Description
- In collaboration with the providers assists with patient advice calls, patient triage assessments, managing clinical boxes, and coordinating EPIC My Portfolio Medical Advice messages.
- In collaboration with the providers completes Medicare Annual Wellness Visits with a goal of 6per week
- In collaboration with the practice manager assists the staff with pre-visit planning to complete tasks associated with closing gaps in care and meeting quality measures.
- Serves as a clinical resource/educator to facilitate the development of the team in collaboration with the provider, practice manager, clinical leads and care manager.
- Reviews quality measures with the Director of the Medical Group, Program Consultant, and Director of Community and Population Health.
Qualifications
- Bachelor’s preferred (or in progress).
- 3 years of clinical nursing experience
- 1 year of care coordination and/or understanding of primary care practice dynamics.
- Current Maryland Nursing License
- Care Management Certification, preferred
- BLS, required
