Shifts: 0730-1530 Monday to Friday

Job Summary
The Clinical Coordinator is responsible for the day-to-day clinical, operational, educational, and quality activities of the dialysis unit. Working collaboratively with the Clinical Manager, APNE, nephrologists, and interdisciplinary team, the Clinical Coordinator ensures the delivery of safe, high-quality patient care, appropriate staffing, staff development, quality improvement initiatives, and effective unit operations.
The Clinical Coordinator provides clinical leadership, serves as a role model and resource for staff, and promotes a culture of safety, accountability, respect, and continuous improvement. The incumbent maintains clinical competence in hemodialysis nursing and is expected to provide direct patient care and assume the Registered Nurse role when required to support unit operations during staffing shortages or unexpected absences.
Duties
- In conjunction with the Clinical Manager, supports patient care delivery and ensures adequate staffing.
- Supports, evaluates, and facilitates Primary Nursing care planning, patient education, and care delivery.
- Participates in quality improvement, patient safety, infection prevention, and operational excellence initiatives.
- Coordinates transplant workups, scheduling of current patients, new patients, travelers, transient patients, and hospital backup patients.
- Serves as a clinical role model, mentor, facilitator, and resource in clinical nursing practice.
- Communicates with feeder hospitals regarding admissions, discharges, transfers, and dialysis capacity.
- Participates in patient assessments and selection processes for admission to the dialysis program.
- Supports transition planning and continuity of care for new and existing patients.
- Maintains and records ORRS and other associated statistical submissions.
- Assists in the development and monitoring of quality indicators and performance measures.
- Coordinates dialysis treatments for international (IPP), out-of-province (OPP), and visiting patients, including documentation, scheduling, payment arrangements, and communication with external partners.
Qualifications
- Current registration with the College of Nurses of Ontario (CNO) in good standing.
- Bachelor of Science in Nursing (BScN).
- Completion of a recognized Nephrology Nursing Program.
- Canadian Nurses Association Certification in Nephrology Nursing (C. Neph.).
- Master's degree in nursing or related field preferred.
- Minimum five (5) years of current hemodialysis nursing experience.
- Demonstrated leadership and coordination experience in a healthcare setting.
- Strong computer skills.
- Demonstrated proficiency with hemodialysis machines (Fresenius), including setup, operation, monitoring, troubleshooting, and treatment management.
- Ability to independently provide direct patient care and assume frontline nursing responsibilities when required.
- Excellent communication, organizational, interpersonal, conflict-resolution, and problem-solving skills.
- EPIC electronic medical record experience preferred.
- Demonstrated teaching, coaching, and staff development experience in hemodialysis preferred.
- Experience leading or participating in Quality Improvement and patient safety initiatives preferred.
- Research experience preferred.
