Wage Range: $45.85 - $57.31 per hour.
Hours: 22.5 Hours per Week
Shifts: 0800-1600
Status: Permanent Part-Time

Position Summary
The Peter Munk Cardiac Centre's Cardiac and Structural Heart Disease Triage offices provide resources designed to improve the healthcare of patients with new and on-going cardiac conditions. The Nursing Clinical Coordinator, in collaboration with the healthcare team, will facilitate patient care, improve the education of the patient and work collaboratively with internal and external healthcare providers to schedule patients for various procedures in the Cardiovascular Investigations Unit (Cath Lab) and Operating Room (OR).
Duties
As a member of the Cardiac and Structural Heart Disease Triage offices, the responsibilities of the Clinical Coordinator, include:
- Utilizes and applies current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities and the coordination of in/out-patient care and continuity in patient care management
- Performs patient teaching/education
- Participates actively in Quality Improvement initiatives
- Participates in academic and research activities/projects
- Performs administrative responsibilities
- Obtains and reviews patient medical records and information
- Liaises regularly with healthcare teams within UHN and throughout Ontario including industry representatives to facilitate treatment for patients with coronary artery disease, heart failure and moderate to severe heart valve disease and other cardiac pathologies
- Schedules patients for various procedures in the cardiovascular investigations unit
- Provides education and support to patients and family members
- Coordination of patient care for inpatients and external referrals
- Performs cross coverage of other Nursing Clinical Coordinator roles and duties as needed
- Other duties as needed to ensure the smooth operation of the Triage offices
Qualifications
- Completion of a recognized BScN (or equivalent)
- Master’s degree, preferred
- Current certificate of competency (licensure) from the College of Nurses of Ontario
- Completion of Basic Cardiac Life Support (B.C.L.S.) program, Level II Rescuer certification
- Minimum Five (5) years’ experience in cardiovascular nursing (cardiovascular surgery or cath lab)
- Minimum Three (3) years’ experience working with patients with moderate to severe heart valve disease
- Cath Lab and or Cardiovascular OR experience, preferred
- Effective organizational, interpersonal and communication skills
- Excellent decision-making and problem-solving skills
- Excellent time management skills
- Excellent teaching skills with the ability to teach one-on-one and/or groups
- Ability to function collaboratively within a multi-disciplinary team and/or independently work in a self-directed manner
- Demonstrated awareness of and adherence to professional and ethical standards
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
- Computer literacy in Word, Outlook, PowerPoint and electronic patient documentation
- Proficient in keyboarding and able to type at least 60 WPM
- Current registration with the College of Nurses of Ontario
- Experience in change management
- Strong organizational and time management skills
- Excellent decision-making and problem- solving skills
- Excellent interpersonal and customer service skills
- Excellent verbal and written communication skills
- Demonstrated ability to cultivate and build relationships
- Strong track record of high- quality work and engagement is essential
- Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities
- Demonstrated ability to use good judgment in assessing difficult situations
- Able to work collaboratively within a group setting and independently - required
- Proven track record of taking initiative
- Computer literacy required - Word Suite and EPIC
- Ambulatory experience preferred
- Experience working with community health providers preferred
