Hours: 37.5 hours per week
Wage range: $49.86 - $62.32 per hour
Status: Temporary Full-Time (until March 31, 2027)

Position Summary
The Advanced Practice Leader provides expert clinical skills and consultation for the care of Multisystem (including oncology, transplant and complex medical patients), Musculoskeletal and Geriatric patients with complex needs, such as multiple physical disabilities, and changes in functional and cognitive abilities.
Duties
- Provide advanced clinical practice consultation for the care of patients with complex needs
- Facilitate the implementation of best practices in relation to the complex care issues of the patient population;
- Engage with patient care team to foster, facilitate, enhance, and support best practice for the Multisystem, Musculoskeletal and Geriatric patient populations
- Work with the patient care team to promote goal-setting and patient-centred care
- Provide coaching and mentorship to enable the evaluation and application of best information in the planning and delivery of care
- Provide leadership in the development, implementation and evaluation of best practices strategy in collaboration with other APLs, educators, and clinical/professional leaders
- Provide leadership to programmatic and corporate initiatives to integrate practice, education and research for best patient and family outcomes
- Publish and present at key peer-reviewed publications/conferences and facilitates staff involvement in publications and presentations
- Promote Toronto Rehab and UHN as an Academic Health Science Centre
Qualifications
- Master's degree in regulated health profession or recognized equivalent
- Licensed/registered member of the profession in current good standing with the regulatory and/or professional body
- At least five (5) years experience and demonstrated expertise in clinical practice relevant to the care of patients in one or more of the following: Multisystem, MSK and Geriatric rehabilitation setting
- Demonstrated knowledge and understanding of the complex and multi-faceted needs (i.e. physical, functional, cognitive. social and emotional) of the Multisystem Musculoskeletal and Geriatric patient populations
- Demonstrated understanding and appreciation of research design, development, implementation, application, evaluation and knowledge translation
- Able to lead and foster research activities with the program and across Toronto Rehab
- Excellent leadership and interpersonal skills with demonstrated ability to foster a supportive work environment and work within inter-professional teams
- Able to effectively foster interprofessional care and collaboration among a diverse patient care team
- Demonstrated facilitation, mentoring, change management and leadership abilities
- Eligibility for cross-appointment to the University of Toronto
