Employee-owned home health company seeking PRN Registered Nurse for the Fort Wayne area.

We are looking for a Registered Nurse to join our Home Health team. The ideal candidate will have experience in home health care, as well as a passion for providing excellent patient care. The successful candidate will be responsible for providing comprehensive nursing services to patients in their homes, and will be required to coordinate with other healthcare professionals to ensure the best possible outcomes. This is an excellent opportunity for a Registered Nurse who is looking to make a difference in the lives of their patients.

Responsibilities:

• Provide direct nursing care to patients in their homes.
• Assess the patient's condition and develop a plan of care in collaboration with the patient and family.
• Provide patient and family education on disease process, medications, and self-care.
• Monitor patient's progress and modify plan of care as needed.
• Document patient's progress in the medical record.

As a Registered Nurse (RN) with Home Health Care Solutions, you are responsible for the nursing care of the patients while in the comfort of their homes.  As a Registered Nurse (RN) you would provide skilled professional nursing care to the patient as prescribed by the physician and in compliance with regulations as established by the Indiana State Board of Nursing and policies of the Agency.  We are currently looking for a motivated and energetic Registered Nurse (RN) to cover the Fort Wayne area.

• Graduate of an accredited school of nursing
• Valid nursing license for the State of Indiana required
• BSN preferred
• One year clinical experience in nursing
• Home Care or Critical Care nursing preferred
