Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities.  The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs.  The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet.  St. Joseph Center serves approximately 6,000 individuals annually.

Brief Description of Principal Activities

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles.  St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

St. Joseph Center also operates a Thrift Store at 1828 Lincoln Boulevard in Santa Monica which offers used items for sale at affordable prices.  The thrift store is open to the public, and some donations to the store are used to provide clients with free clothing.  Proceeds from the store help support  the programs and services described above. 

Job Description

GENERAL DESCRIPTION:  


Under the supervision of the Program Manager, the Rapid Rehousing/Retention Services Case Manager will provide field-based services to homeless individuals who are enrolled in our new Rapid Rehousing Program. This program offers short-term rental subsidies combined with linkages to employment and other services in order to end clients’ homelessness as well as create longer term self-sufficiency. Once clients are stably housed through the support of the Case Manager and Housing Specialist, the Case Manager will also be responsible for providing ongoing services that will assist clients to retain their housing and be good tenants and neighbors. Each Case Manager will be assigned 20-30 clients depending on other responsibilities. 


RESPONSIBILITIES: 

1. Develop and implement an individualized service plan with clients that supports housing goals, increases income and mainstream benefits, supports employment opportunities, and addresses mental health/substance abuse issues, relapse prevention, medication management and any other risk factors that could impede accessing and/or maintaining permanent housing.

2. Support clients as they navigate the housing process, including, but not limited to, housing location, landlord advocacy, rental application process, credit repair support, move-in support, and budgeting.  

3. Develop close working relationships with landlords, local service providers, outreach workers, case managers, and collaborative agencies.

4. Provide ongoing case management services that will ensure successful permanent housing, decrease social isolation and reduce relapse risks, advocacy to deter evictions, and linkage and brokerage to any needed services such as medical/dental, mental health, substance abuse, life skills training, self-help, money management, meaningful community activities, volunteer services, job placement, etc.

5. Utilize a harm reduction/housing first model when working with clients. 

6. Respond to urgent requests for assistance from clients or landlords as needed.

7. Assist client with re-housing/ moving when needed.

8. Keep thorough and high-quality case notes and enter in HMIS within 24 hours. 

9. Complete all forms required for rapid rehousing thoroughly and completely. 

10. Obtain and retain all required documentation (e.g., leases, income verification) as required by the program.  

11. Participate in all meetings with collaborative agencies and other community meetings as directed.

12. Complete all requests for reporting in a timely manner.




Qualifications

QUALIFICATIONS: 

Candidates should possess a bachelor’s degree or have equivalent experience including one year’s experience providing services to persons experiencing homelessness. Candidates must also demonstrate experience in interviewing clients, making appropriate referrals and performing crisis intervention. Previous experience with rapid rehousing programs including HPRP, SSVF or other similar programs preferred.


Candidates must demonstrate knowledge of substance abuse programs, homeless programs, and mental health services within the Los Angeles Continuum of Care. Candidates must have demonstrated knowledge of case management principles and intervention techniques specific to hard-to-reach, difficult to serve populations. 


The position requires an ability to work flexible hours including some early mornings, evenings. A bilingual/bicultural background is a plus.
