Job Description

The Intake Coordinator - Home Medical Equipment reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Under the direction of the Director of Home Medical Equipment, the Intake Coordinator is responsible for receiving referrals requesting the level of service in the continuum of care. The Intake Coordinator is responsible for gathering information of new referrals, identifying Home Medical Equipment needs of patients, obtaining orders from the patient's physician, and beginning the smooth transition from the hospital environment to the home setting.

Responsibilities:

  • Review the patients chart to obtain demographic, insurance and relevant clinical information.
  • Verify the orders and health insurance benefits meet the qualification requirement to receive Home Medical Equipment according to Medicare, Medicaid and third party payer guidelines.
  • Has direct contact with Physicians, Nurses, and Discharge planners to coordinate patient needs and the delivery of Home Medical Equipment in a timely matter.
  • Negotiates contracts/payments with third party vendors not contracted with CHS if the patient meets criteria for Home Medical Equipment services.
  • Integrates the CHS Mission, Vision, and Values into each of the responsibilities of this job and daily communication with our customers and co-workers.
  • Adheres to and is responsible for the safety standards of Centegra Health System including the completion of the mandatory organizational safety standards on an annual basis.
  • Actively maintains confidentiality for our patients and their families as outlined in the CHS Patient Confidentiality Policy and shows the same level of respect for every Associate with in CHS.
  • Provides telephone intake of information relating to patient referrals.
  • Prepare and secure Certificates of Medical Necessity (CMNs).
  • Determines appropriateness of referral to home care based on established criteria.
  • Disseminates the intake information to appropriate sources.
  • Assigns third party vendors based on needs of patient.
  • Obtains/receives physician orders.
  • Enters referral and other appropriate information into the computer.
  • Assists HME Director with other duties as needed.
  • Attend in-services and continuing education programs when appropriate and assigned.
  • Remain current on innovations, developments, new procedures, and techniques in nursing care as it may pertain to clinical referral information.
  • Actively participates in system development and supports staff participation throughout the department.
  • Performs other duties as assigned.

Qualifications

Required:

  • High school diploma or equivalent.
  • Strong communication skills in speaking and writing.
  • HME Insurance requirements knowledge and HME intake experience.
  • Physical ability to perform functional requirements as detailed.
  • Computer skills.
  • Must display creativity, vision, flexibility, organizational skills, ability to suggest innovative solutions to problems.
  • Must display good verbal and written communication skills.

Preferred:

  • Previous HME customer service experience.
  • Computer experience.
  • Experience with Brightree Software.
