The Patient Education Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The Patient Education Coordinator is responsible for planning, developing, coordinating, reviewing and evaluating patient/family education processes, guidelines and materials.

The schedule for this position is Monday-Friday 7am-3:30pm with some flexibility required.

Responsibilities:

• Develops educational processes and procedures to assure congruence with regulatory and accrediting body standards as well as consistency across the inpatient/outpatient care continuum.
• Collaborates with clinicians and clinical leaders to maximize the value of educational opportunities which enhance the patient experience.
• Provides patient education consultation to managers, unit-based staff, and other interdisciplinary healthcare professionals using in-depth clinical knowledge, skills and expertise across the continuum of care.
• Employs variety of methods in assessment of learning needs of patients, family and others supporting the patient.
• Develops, selects and recommends materials and programs for use in educating the patient, family and other support persons to improve patient health outcomes.
• Interacts professionally with the nursing staff and nurse leadership to determine specific educational needs of patients and their families or support persons, and to coordinate educational programs and materials to meet those needs.
• Recommends and ensures materials appropriate to teach patients/families over the life span; pediatric, adolescent, adult and the geriatric population.
• Assists and coaches other staff in development, implementation and evaluation of teaching plans that address patient needs, abilities, preferences and readiness to learn.
• Provides quality, comprehensive and timely review and/or development of patient educational materials and resources, ensuring appropriate health literacy level.
• Coordinates Patient Education Advisory Council.
• Coordinates meetings and program logistics in support of patient education performance improvement initiatives.
• Promotes patient education through writing, publishing and presentations.
• Assists with developing a budget for patient education programs and with writing grant proposals as necessary.
• Utilizes research based knowledge and clinical experience in decision making. Stays current regarding trends, new technology/development, teaching methodologies through professional journals, seminars/conferences, professional societies, etc.
• Collaborates with other NM system professionals to coordinate and advance patient education outcomes.
• Facilitates compliance with NM, regulatory and accreditation standards related to patient education in partnership with Professional Development Director and reports variances to leadership as appropriate.

Required:

• Master’s Degree in Nursing or related field or currently enrolled
• Licensed Registered Nurse in the state of Illinois
• 3 years clinical nursing experience
• Knowledge of adult learning principles
• Able to read, write and communicate effectively in English
• Excellent written and verbal communication skills
• Proficient in Word, Excel, PowerPoint and Adobe applications

Preferred:

• Five years clinical nursing experience
• Previous or related experience developing or evaluating patient education materials and strategies
