Job Description

The BCVI Clinical Nurse Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The BCVI Clinical Nurse Coordinator is a registered professional nurse who will coordinate care for patients within the cardiovascular program including interventional cardiology and structural heart. The responsibilities and duties include serving as the point of contact for referring providers, processing new patient referrals, and organizing clinical conferences and meetings. This position coordinates screening, additional testing requirements, consult appointments, and hospital-based procedures. Facilitates communication with cardiologists, cardiovascular surgeons, and primary care physicians, including referring physicians regarding patient status for procedures.

The BCVI Clinical Nurse Coordinator will utilize and manage available data to help drive clinical workflows or work plans by organizing, documenting and presenting the information received and reviewing it with the cardiovascular team to determine appropriate patient care pathways.

Under the supervision of the BCVI Clinical Program Manager, the BCVI Clinical Nurse Coordinator will coordinate care for structural heart patients demonstrating the skills below:

  • Establishes and maintains a courteous and professional support with patients, family members, visitors, co-workers, and referring physicians.
  • Excellent interpersonal and communication skills required for patient education.
  • Knowledge of valve and other cardiac surgeries and procedures
  • Possesses computer skills to prepare spreadsheets and enter data.
  • Assuring that the documentation of care and the patient’s follow-up information is available in the medical record.

Responsibilities:

  • Serves as primary point of contact for outpatient referrals, responsible for the intake of patient referrals for evaluation and candidacy for minimally invasive structural heart procedures including but not limited to TAVI, TEER and Complex Coronary, and reviewing with the Structural Interventional Cardiologists as indicated.
  • Collaborates with the Cardiology Patient Navigator, whom responsibilities include managing messages for new patients, and established patients, managing and maintaining schedule templates for outpatient clinic, and serves as primary point of contact for all scheduling requests, questions, and administrative issues.
  • Administratively supports the pre-procedure work-up. Establishes contact with patient/family to explain process and workup. Obtains all outside imaging, schedule/order pre-procedure testing, facilitates dental clearance, arranges cardiology clinic appointments, CV surgery appointments, other consultations as needed and structural heart APN appointments. Coordinates with multiple departments to schedule pre-operative testing and appointments for patients. Verifies the status of pre-procedure testing to ensure all necessary tests are completed and results are available for providers.
  • Schedules and coordinates clinical conferences as needed. Prepares and organizes all meeting content, sets agenda and creates PowerPoint presentations to be discussed.
  • Utilizes available sources of data to identify and track patients with structural heart conditions to ensure appropriate care plans for patients.
  • Responsible for educating patients and families on pre-operative and post-operative care, procedural expectations, follow-up appointments, medications, additional testing, or any aspect of their care as needed.
  • Other BCVI Clinical Nurse Coordinator duties as assigned.

Qualifications

Required:

  • Bachelor’s Degree in Nursing
  • Current IL nursing license
  • Current Basic Life Support certification
  • Minimum of 5 years experience in Cardiovascular surgery and/or Cardiology
  • Experience working in hospital or clinic setting with direct Physician interaction

Preferred:

  • Experience with appointment or procedure setting.
  • Proficient in Excel, PowerPoint, and Microsoft Word
