Monday - Friday 

Hours: 8:30am to 5:00pm (flexible)

Markets community based medicine resources, including identification of patients for community based medicine

This role works in an office setting consisting in part of triaging phone calls

Working in a team for a home based house call program

Collaboration with Nurse practitioners

Current license in the State of Illinois

BSN preferred

Experience in home care, hospice or physician office preferred

Triage experience preferred
