The care and support we provide is highly valued by the people we care for and their loved ones, but at present we are only reaching around 10% of dying people at the end of life. Right now, one in four people in the UK with a terminal illness, do not get the care or support they deserve at the end of their lives.  

Job Description

We are recruiting a Healthcare Assistant to join our new Care Homes Advance Care Planning Service in Barnsley.

Salary: Marie Curie Clinical Pay Scale Band 2 (aligned with AFC) £24,465 per annum (increasing to £25,272 from 1st April 2026) + enhancements (pro rata)

Contract/Hours:   Part Time – 22.5 hours per week

Contract Type: Permanent

Based: Silkstone Health Centre, Barnsley

This is an excellent opportunity to join our Care Homes Advance Care Planning Service in Barnsley and make a real difference to people at an important time in their lives.

Based in primary care and working in partnership with Silkstone Health Centre, you’ll collaborate closely with GP's to support patients living in local care homes. Using your experience in palliative care, you’ll help patients and their families engage in meaningful advance care planning.

Working under the line management of our Clinical Nurse Specialists, you’ll provide direct, compassionate support to patients and their families, building trusted relationships across community and primary care services. This role is ideal for a confident and empathetic communicator with a strong understanding of palliative and community care.

As this is a community‑based role, you’ll be required to visit patients in their care homes, therefore driving and access to your own vehicle for business use is essential.

What we’re looking for:  

  • S/NVQ Level 2/3 qualification or an equivalent certificate/diploma is required  
  • Relevant community experience, providing care and support for people with life limiting illness or palliative care needs
  • Advanced communication skills with the ability to have difficult and sensitive conversations.
  • Emotional resilience and the ability to work on your own are crucial, though we will of course provide excellent on-going training and support  
  • Be a UK licensed driver with access to a car and business insurance is essential 
  • This position is funded by the Oak Foundation Service for an initial period of two years, with the potential for funding to be extended
