Job Description

Permanent Supportive Housing Case Manager (PSHCM) will provide housing and support services to chronically homeless families and individuals in the servicing community. The Permanent Supportive Housing Program utilizes the Housing First approach in responding to the needs of vulnerable families and individuals experiencing homelessness. The Housing First approach is the commitment to provide housing to those experiencing homelessness as quickly as possible without creating prerequisites to being housed, such as participation in drug treatment programs. This position must be staffed with a dedicated and service-oriented individual, who is committed to working in a professional environment with at-risk communities.

 

Responsibilities:

  • Provide overall client support and engagement activities to meet immediate and long-term needs in a caring and programmatic manner.
  • Support outreach and engagement of families /individuals and facilitating the client’s permanent supportive housing needs with the Department of Humans Services.
  • Assist in processing housing applications, lease agreements with landlords, families and contractors when needed.
  • Conduct initial visits to units and assist clients with unit inspections as needed to ensure appropriate requirements and compliance with program regulations.
  • Work with families and individuals to develop service plans, set client goals and implement strategies to meet goals. Track and report on progress of goals regularly.
  • Work with families and individuals to complete a bio-psycho-social for housed consumers.
  • Conduct ongoing home visits to assess client needs and make referrals as appropriate.
  • Actively network with the community service providers and other partner agencies to locate programs and resources to assist families.
  • The case manager shall engage the consumers minimum of 4 face-to-face contacts monthly.
  • Input client data into Housing and Homeless (HTH) databases. Produce client reports and generate checks to clients for financial assistance.
  • Establish and maintain weekly meetings, based on client assessments
  • Maintain an organized, descriptive and up-to-date record for each client according to DHS requirements.
  • Report issues related to consumer case management needs to Team Lead ad Program Manager.
  • Provide consumer data for weekly and monthly reports.
  • Attends all company- and DHS required training.
  • Attend mandatory supervision.
  • Attend mandatory assigned/scheduled office day.
  • Assist consumers with ADL skills as needed.
  • Other relevant duties as assigned.

Qualifications

 

  •  Two to six years experience working within the homeless field (required).
  •  Bachelor's Degree
  • Crisis management experience (preferred).
  • Excellent written and oral communication skills.
  • Excellent time management skills.
  • Exceptional ability to multi-task and excellent organizational skills.
  • Excellent case management skills with a proven track record of success.
  • Prior experience and understanding of causes of homelessness.
  • Experience working with landlords, local rental markets and/or housing sales.
  • Ability to work in a team-oriented environment.
  • Ability to work with as well as serve a diverse population.

Additional Information

Salary Range: $52,000 - $55,000
