This position allows you to work from home, providing support to LTSS MMP HIV/TBI waiver participants in Christian, DeWitt, Logan, Macon, and Sangamon counties. We are currently offering a $3,000 sign-on bonus for this role.
In this role, you will travel approximately 75% of the time, with mileage reimbursement based on the current IRS guideline rate.
Position Purpose: Your main responsibility will be to ensure that our members have access to the services they need by coordinating and integrating medical and long-term care services. This includes conducting assessments, developing care plans, and coordinating care.
Your key responsibilities will include:
- Conducting assessments with members, caregivers, or providers to gather information on client status, support system, and service needs for care plan development
- Monitoring service delivery and following up with members, caregivers, or providers through in-person visits and phone calls
- Authorizing and coordinating referrals for services
- Ensuring that provider services are delivered without gaps and identifying any deficiencies in care plans
- Assisting in the coordination of informal or voluntary services to integrate into the member's care plan
- Collaborating with discharge planners, physicians, and other parties to ensure appropriate discharge planning and coordination of acute and long-term care services
- Assisting members with filing and resolving complaints and appeals
Education/Experience: To be considered for this position, you should have at least a Bachelor's degree or be a Registered Nurse with 2+ years of care management experience. Alternatively, you can be an LPN with 4+ years of care management experience, or have 6+ years of care management experience. Experience in home health, discharge planning, or long-term care is highly preferred.
Brain Injury/HIV/AIDS: To work with this specific population, you must meet one of the following criteria:
- 1. A Registered Nurse (RN) licensed in Illinois with a bachelor's degree in nursing, social work, social sciences, counseling, or four (4) years of case management experience
- 2. Be a certified or licensed social worker with a Bachelor's degree in social work, social sciences, or counseling, or hold a master's degree in social work
- 3. If you're an unlicensed social worker, you must have a minimum of a bachelor's degree in social work, social sciences, or counseling
In addition to meeting one of the above criteria, you should have experience working with:
- Addictive and dysfunctional family systems
- Racial and ethnic minorities
- Homosexuals and bisexuals
- Persons with AIDS
- Substance abusers
