Position: Supervisor of Care Management
As a Supervisor of Care Management, you will play a vital role in implementing our Care Management programs in multiple environments. You will be responsible for ensuring the continuity of care for our members and assisting them in accessing the healthcare services they need. Your role will involve supervising a team, assessing member needs, developing care plans, and coordinating resources for effective care management.
Responsibilities:
- Supervise and support a team of care managers, including hiring, training, and performance evaluations.
- Triage and assess members at risk to identify individual needs.
- Develop comprehensive care management plans based on member assessments.
- Coordinate activities to achieve the goals set forth in the care management plan.
- Manage resources and ensure continuity of care for members.
- Collaborate with Medical Informatics and Quality Improvement departments to monitor program effectiveness.
- Evaluate the effectiveness of care management plans and make necessary adjustments.
- Identify areas of opportunity and provide education to providers as needed.
- Maintain relationships with care management vendors to ensure compliance with policies and regulations.
- Communicate with providers, members, and community resources to support patient care planning.
- Participate in audits and contribute to corrective actions if needed.
- Perform care management activities when required.
Requirements:
- CCM certification or Baccalaureate degree (MSW with care management experience may be considered).
- Five years of clinical experience, including two years of supervisory experience.
- Current, valid, and unrestricted professional licensure (RN preferred).
