Join Our Team as a Registered Nurse (RN) Case Manager

As an RN Case Manager, you will be the point of contact for all disciplines involved in patient care. Your responsibilities include performing visits, coordinating care, consulting with physicians, and maintaining accurate records. To qualify for this role, you must:
