As a healthcare provider at Enhabit, you will provide medical care to hospice patients. Your responsibilities will include determining hospice eligibility, managing pain and symptoms, conducting history and physical exams, diagnosing and treating medical problems, and providing education and counseling to individuals, families, and groups as part of a interdisciplinary team. You may also perform face-to-face patient visits to assist in determining hospice eligibility and, if chosen by the patient or representative, become the attending physician.

Requirements:

• Graduate of an approved school
• Licensed as a nurse practitioner in the state of practice
• Basic technology skills, including use of a mobile device

Preferred Education and Experience:

• At least five years of nursing experience, with one year in hospice preferred
• At least three years of clinical registered nurse practitioner experience preferred

Requirements:

• Valid state driver's license
• Automobile liability insurance
• Dependable transportation in good working condition
• Ability to safely drive in all weather conditions
• CPR certification for healthcare provider
