Job Position: Home Health Registered Nurse (RN)

Job Summary:

The Registered Nurse (RN) provides skilled care to select patients by assessing and monitoring their physical condition, providing patient and caregiver education, and performing treatments and procedures based on physician orders. The RN helps patients achieve and maintain their optimal level of function and provides the necessary care in a home and community environment.

Responsibilities:

The RN's responsibilities include:

  • Providing care for the physical, mental, developmental, emotional, and social needs of patients, regardless of age.
  • Evaluating the condition and status of each patient, including physiological, cognitive/mental, developmental, emotional, and social aspects.
  • Administering treatments and procedures as directed by the physician's plan of care.
  • Offering patient and caregiver education when required.
  • Assisting in updating comprehensive assessments when necessary due to changes in the patient's condition or regulatory follow-ups.
  • Helping patients achieve and maintain their optimal level of function.
  • Assisting with personal care and activities of daily living (ADLs) to ensure proper hygiene and nutrition.
  • Contributing to the patient care team by providing input on the patient's plan of care and progress towards goals.
  • Maintaining a clean working and patient care environment.
  • Documenting all treatments and care provided, as well as patient responses to treatments.
  • Reporting any changes in the patient's condition to physicians, Case Coordinators, and Clinical Supervisors.
  • Obtaining or clarifying home care orders and communicating changes to the team.
  • Ensuring a safe environment and reporting any safety concerns to the appropriate personnel.
  • Providing medical clinic nursing coverage at the Summit County Juvenile Detention Center as assigned.
  • Performing other duties as required.

Requirements:

The ideal candidate should possess:

  • Experience in home health or acute care settings (preferred).
  • Experience in Pediatrics and/or working with individuals with developmental delays (preferred).
  • Experience in managing technology-dependent and/or medically complex patients (preferred).
  • Effective communication skills to work with individuals at all levels within the organization.
  • Proficiency in MS Office (Outlook, Excel, Word) or similar software.
  • Educational requirements: Graduation from an approved accredited Registered Nursing program.
  • Certification: Current Ohio licensure as a Registered Nurse and current CPR/BLS certification through the American Heart Association.
  • Experience: Six months of experience preferred in areas such as Home Care, Geriatric Care, Medical/Surgical, CCU, Pediatrics, Obstetrics, or Department of Developmental Disabilities (DODD).
  • No prior supervisory experience required.
