Position Purpose:

We are seeking individuals who can assess the needs of our members and develop service plans that align with their goals and preferences related to employment, housing, participant direction, independent living, and community transitions. As a subject matter expert, you will support Service Coordinators and Care Managers in creating person-centered service plans.

Responsibilities:
  • Conduct assessments with members, caregivers, and/or providers to gather information about their status, support system, and service needs.
  • Connect members to local resources for employment, housing, participant direction, independent living, and community transitions based on assessments and service plans.
  • Authorize and coordinate referrals for services.
  • Monitor service delivery and maintain regular contact with members, caregivers, and/or providers through in-person visits and remote communication.
  • Participate in Advisory Committee and Interdisciplinary Care Team meetings.
  • Develop and update health plan resources in assigned focus areas such as accessible housing units and employment directories.
  • Track progress and activities related to assigned focus areas, supporting health plan performance goals and quality initiatives.
  • Build relationships with providers, community-based organizations, and relevant governmental entities to ensure members and families can access essential services and resources.
  • Provide technical expertise and training to the health plan as needed on regulations, benefits, and rules in assigned focus areas.
Requirements:
  • Bachelor’s degree in a related field or equivalent experience.
  • Minimum 2 years of experience in employment, housing, transitions, or participant direction programs.
  • Experience working with LTSS, chronic or complex condition populations is preferred.
  • Must have a valid State Driver’s License.

Note: Candidates must reside in Oklahoma.
