This position is remote, allowing you to work from home and support LTSS MMP HIV/TBI waiver participants in Christian, DeWitt, Logan, Macon, and Sangamon Counties. We are also offering a $3,000 sign-on bonus for this role. Please note that you will be required to travel 75% of the time, and we will reimburse your mileage at the current IRS guideline rate.
Position Purpose: Your role will be to ensure our members have access to the services they need by coordinating and integrating medical and long-term care services. This includes conducting assessments, developing care plans, coordinating services, and providing care coordination.
In this role, you will:
- Complete assessments with members, caregivers, or providers to gather information about the client's status, support system, and service needs for care plan development.
- Monitor the delivery of services and follow up with members, caregivers, or providers through in-person visits and phone calls.
- Authorize and coordinate referrals for services.
- Ensure that provider services are delivered without gaps and identify any deficiencies in the care plans.
- Assist in integrating informal or voluntary services into the member's care plan.
- Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge and care plans, as well as coordination of acute care and long-term care services.
- Assist members in filing and resolving complaints and appeals.
Education/Experience: To be considered for this position, you should have at least a Bachelor's degree or be a registered nurse with 2+ years of care management experience. If you are an LPN, you should have 4+ years of care management experience, and if you do not have a degree, you should have 6+ years of care management experience. Home health, discharge planning, or long-term care experience is preferred.
Requirements for Brain Injury/HIV/AIDS: You must meet one of the following criteria:
- 1. A Registered Nurse (RN) licensed in Illinois with a Bachelor's degree in nursing, social work, social sciences, or counseling, or four (4) years of case management experience.
- 2. Certified or Licensed social worker with a Bachelor's degree in social work, social sciences, or counseling, or a Master's degree in social work.
- 3. Unlicensed social worker with a minimum of a Bachelor's degree in social work, social sciences, or counseling.
