Summary of Essential Duties:

• Serve as a clinical co-pilot, taking care of each patient's medical, psycho-social, financial, and practical needs.
• Coordinate the care and service of patients from admission to discharge.
• Assess, plan, implement, monitor, and evaluate options and services to meet individual health needs and facilitate a safe and timely discharge.

Educational Requirements:

• Preferred: Bachelor's Degree in Nursing (BSN)
• Required: Associate's Degree in Nursing (CA License)

Licenses:

• Preferred: Experience and certification in Case Management
• Expected within 2 years of hire: Case Management certification
• BLS Certified

Experience:

• Minimum of 3 years in Acute Care Nursing
