The Registered Nurse (RN) Case Manager plays a crucial role in coordinating care for our patients. They are the main point of contact for all healthcare disciplines involved and ensure that visits are scheduled appropriately. The RN Case Manager also works closely with physicians and communicates necessary patient care details. Other responsibilities include accurate OASIS collection, medication management, timely reporting of lab values, and participation in case conferences and meetings.

This part-time salaried position requires working 24 hours per week over 4 days and is eligible for our 60% paid days off program.

Registered Nurses (RNs) must meet the following requirements:

1. Current Registered Nurse (RN) license in the state of employment
2. Preferred minimum of one year of clinical experience
3. Knowledge and skills in current nursing practice
4. Valid state driver’s license and automobile liability insurance
