About the Position:As a clinical professional, your role will be to assess, plan, and coordinate all aspects of medical and supporting services for our members. Your goal will be to provide high-quality and cost-effective care for our members through comprehensive care management.
- Develop and adjust care plans to promote desired outcomes
- Assess current health status, treatment plans, and resource utilization
- Coordinate services between healthcare providers to meet members' needs
- Provide patient and provider education
- Facilitate access to community-based services
- Monitor referrals and support members' overall care management plan
- Collaborate with integrated team care management rounds
- Identify and report risk management concerns
- Maintain accurate and up-to-date clinical information

- Bachelor's degree in Social Work
- 2+ years of social work experience in acute care or community setting
- Knowledge of government-sponsored managed care programs is preferred
- Current state's LMSW or LCSW license
If you are applying for Sunshine Health in Florida, additional requirements include:
- Must have an RN, LCSW, LMSW, LMHC, or other Mental Health Professional license
- Minimum of two years of pediatric experience for Florida licensed registered nurses or a Master's degree in social work or related field with one year of related professional pediatric care experience
- Up to 80% local travel may be required
- Licensure requirement: Master's degree in Social Work and LCSW, LMSW, LMHC
- Experience: 1 year (minimum) pediatric experience
- Location: Residency required in Miami-Dade County
- Travel: 80% local travel
