Role and Responsibilities: Join our Compassionate Team

• Perform in-home patient visits and ensure the visit time meets the patient's needs
• Coordinate total patient care and communicate with other healthcare providers to enhance continuity of care
• Update the patient's plan of care and report significant changes to the physician and branch staff
• Guide and instruct patients and families in activities to improve independence and functionality
• Review and update medication profiles and assess patient compliance and understanding
• Complete various assessments and discharge procedures according to established protocols
• Accurately and timely document all necessary information in electronic medical records

Qualifications: Join our Diverse and Inclusive Team

• Must be a licensed Registered Nurse (RN) in the state of employment
• Minimum one year of RN experience
• Previous Medicare home health or hospice experience is preferred
• Must possess a valid state driver's license and maintain automobile liability insurance
• CPR certification for healthcare providers
