Position Purpose: We are looking for a clinical professional who will assess needs and develop service plans for our members based on their goals and preferences. This includes employment, housing, participant direction, independent living, and community transitions. You will serve as an expert in supporting the development of person-centered service plans for our members, working closely with Service Coordinators and Care Managers.

Your responsibilities will include:

  • Conducting assessments with members, caregivers, and providers to gather information about their status, support system, and service needs
  • Connecting members with local resources for employment, housing, participant direction, independent living, and community transitions based on their assessments and service plans
  • Authorizing and coordinating referrals for services
  • Monitoring service delivery and following up with members, caregivers, and providers through in-person visits and telephonic contact
  • Participating in Advisory Committee and Interdisciplinary Care Team meetings
  • Developing and maintaining health plan resources in assigned areas, such as accessible housing units and employment directories
  • Tracking activities and progress in assigned areas to support health plan performance goals and quality initiatives
  • Building relationships with providers, community-based organizations, and relevant governmental entities to help members and their families access essential services and resources
  • Providing technical expertise and training to the health plan on rules, regulations, and benefits in assigned areas as needed

Education/Experience: You should have a Bachelor's degree in a related field or equivalent experience. You should also have 2+ years of experience in employment, housing, transitions, or participant direction programs. Experience working with LTSS, chronic, or complex condition populations is preferred. Due to the nature of this role, you will be expected to travel at least 25% of the time.

Licenses/Certifications: A valid State Driver's License is required.

Candidates must live in Oklahoma.
