This position allows you to work remotely from home and support LTSS MMP HIV/TBI waiver participants in Christian, DeWitt, Logan, Macon, and Sangamon counties. You will be required to travel 75% of the time, and we provide mileage reimbursement at the current IRS guideline rate.
Position Purpose: Your role will involve coordinating and integrating medical and long-term care services to ensure our members have access to the care they need. This includes assessing client status, developing care plans, and coordinating care.
- Meet with members, caregivers, or providers to gather information for care plan development
- Monitor service delivery and follow up through in-person visits and phone calls
- Coordinate and authorize referrals for services
- Identify any gaps in care plans and ensure seamless delivery of services
- Help integrate informal or voluntary services into member care plans
- Collaborate with discharge planners, physicians, and other parties to coordinate acute and long-term care services
- Assist members with complaints and appeals
Education/Experience:
We require a Bachelor's degree or Registered Nurse licensure with at least 2 years of care management experience. Alternatively, you can have an LPN license with at least 4 years of care management experience, or a minimum of 6 years of care management experience. Experience in home health, discharge planning, or long-term care is highly preferred.
Brain Injury/HIV/AIDS:
- Option 1: Registered Nurse (RN) license in Illinois and a bachelor's degree in nursing, social work, social sciences, or counseling OR minimum of 4 years of case management experience
- Option 2: Certified or licensed social worker with a bachelor's degree in social work, social sciences, or counseling OR a Master's in social work
- Option 3: Unlicensed social worker with a bachelor's degree in social work, social sciences, or counseling
