Summary
The HIV Health Navigator provides comprehensive support, guidance, and care coordination for individuals living with HIV or those at high risk of HIV infection. The navigator helps clients access medical care, treatment, prevention services, community resources, and social support programs while promoting adherence to treatment plans and improving health outcomes.
Key Responsibilities
- Assist clients in accessing HIV-related medical care, including primary care, specialty care, and treatment services.
- Conduct client needs assessments and develop individualized care plans.
- Educate clients about HIV prevention, treatment, medication adherence, and available support services.
- Coordinate referrals to healthcare providers, mental health services, substance use treatment, housing programs, transportation services, and financial assistance resources.
- Monitor client progress and follow up to ensure engagement in care and successful completion of referrals.
- Provide advocacy and support to reduce barriers to healthcare access.
- Maintain accurate and timely documentation in electronic health records and case management systems.
- Collaborate with physicians, nurses, social workers, case managers, pharmacists, and community organizations.
- Assist with linkage-to-care and retention-in-care initiatives.
- Support clients in navigating insurance benefits, including Medicaid, Medicare, Ryan White Programs, and other assistance programs.
- Participate in outreach activities, community education events, and HIV prevention initiatives.
- Ensure compliance with confidentiality requirements, HIPAA regulations, and organizational policies.
Qualifications
Required Qualifications
- Bachelor's degree in Social Work, Public Health, Psychology, Human Services, Nursing, or a related field preferred.
Preferred Qualifications
- Certification as a Community Health Worker (CHW), Patient Navigator, or HIV specialist.
- Experience with Ryan White HIV/AIDS Program services.
- Bilingual skills, preferred.
- Knowledge of trauma-informed care, harm reduction principles, and cultural competency practices.
- Experience working with individuals living with HIV/AIDS, underserved populations, or in healthcare/social service settings.
- Knowledge of HIV disease management, treatment options, prevention strategies, and community resources.
- Strong communication, counseling, and interpersonal skills.
- Ability to build trust with diverse populations.
- Experience with case management and care coordination.
- Proficiency with electronic medical records and Microsoft Office applications.
- Valid driver's license and reliable transportation may be required.
