Role Overview

As a Case Manager, you¿ll be a vital member of our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. Your role is to identify, assess, and plan care strategies that promote safety, quality, and cost-effective outcomes. You¿ll facilitate transitions to the next level of care, manage critical diagnoses, and optimize health outcomes across the care continuum, making a direct impact on patient wellbeing.

What You Will Do

  • Collaborate with interdisciplinary teams to assess patient needs and develop individualized care plans.
  • Coordinate discharge planning to ensure safe transitions to the next level of care.
  • Advocate for patients and families by connecting them with community resources and support services.
  • Evaluate care outcomes and adjust strategies to optimize health and wellness.
  • Promote patient safety, quality of care, and cost-effective solutions across the care continuum.

 

Qualifications

What You Will Need

Required Qualifications

 

  • Bachelor's degree in nursing or Master's degree in social work.
  • Minimum 3 years of clinical experience.
  • Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Master Social Worker (LMSW).
  • Excellent verbal communication, written documentation, and interpersonal skills.
  • Strong problem-solving, analytical, and decision-making abilities.
  • Proficiency with computer systems and healthcare software applications.

 

Preferred Qualifications

 

  • Certification in Case Management (CCM) or Accredited Case Manager (ACM).
  • Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
