In December 2024, we began providing health, mental health, and wellbeing services across the onshore Immigration Detention network. Our team of nurses, general practitioners, and allied health professionals offer a range of healthcare services, including health assessments, chronic disease management, health promotion, wound management, immunizations, and other primary health interventions.  HCA also launched a 24/7 Virtual Care Centre (VCC), offering remote healthcare through telehealth and video consultations to enhance onsite care for detainees 

Job Description

As a Virtual Triage Nurse, you will be responsible for conducting comprehensive clinical assessments via telehealth platforms, prioritising patient care based on clinical urgency, risk, and presenting needs. You will play a key role in coordinating appropriate care pathways, ensuring patients are directed to the right service at the right time.

You will also provide patient education, support self-management strategies, and contribute to positive health outcomes through effective communication and clinical guidance.

Day to day! 

  • Conduct virtual clinical assessments to identify patient needs across acute and chronic presentations
  • Prioritise patients based on clinical urgency, risk factors, and severity of symptoms
  • Collect and document accurate patient histories and clinical information in electronic health records
  • Coordinate care pathways in collaboration with GPs, Nurse Practitioners, specialists, and allied health providers
  • Facilitate referrals to appropriate services, diagnostics, and follow-up care
  • Provide patient education to support self-management of chronic conditions such as diabetes, hypertension, and respiratory illness
  • Support patients in understanding their care plans, treatment options, and next steps
  • Monitor patient progress and follow up where required to ensure continuity of care
  • Maintain accurate, timely, and compliant clinical documentation
  • Work collaboratively within a multidisciplinary virtual care team
  • Contribute to continuous improvement and quality care initiatives

Lets talk about you!

You are a confident and experienced Registered Nurse with a Bachelor of Nursing (or equivalent) and current unrestricted AHPRA registration, bringing a minimum of 5 years’ clinical experience in primary care, triage, assessment, or case management. You are highly autonomous, thrive in fast-paced virtual environments, and demonstrate strong clinical judgement, communication, and interpersonal skills, with proficiency in electronic health records and telehealth platforms. You also bring valuable experience in primary health, community or urgent care settings, with exposure to complex and chronic disease management, and ideally have experience working in immigration detention or similar environments.
