Job Description
As a Care Planner within our Short Term Care at Home Service, you’ll play a vital role in ensuring our service users receive safe, effective, and person-centred care.
What you’ll do:
- Complete and review care plans tailored to individual needs
- Carry out risk assessments to ensure safe delivery of care
- Work closely with care teams to maintain high standards
- Support service users transitioning through short-term care
- Ensure all documentation is accurate and up to date
- Contribute to delivering high-quality, outcome-focused care

Qualifications

- Organised and detail-oriented
- Compassionate and person-centred in approach
- Confident working independently and as part of a team
- Experienced in care planning and risk assessments (preferred)
