Job Description

Director of Nursing, Registered Nurse (DON) — Home Care
Pay Range: $90,000–$102,000

At Rhythms Home Care, we believe older adults deserve to thrive, not just be cared for. We're looking for a Director of Nursing who leads with heart and holds our values close: belonging, celebration, empowerment, and the unique skills each person brings to our team.

What you'll do:

  • Be the clinical heartbeat of our home care team — leading assessments, care planning, and skilled nursing with warmth and precision
  • Build real relationships with clients, families, and physicians so every person feels seen and supported
  • Lead, mentor, and empower our direct care staff (CNAs, PCAs) — recruiting, training, and helping them grow into their strengths
  • Ensure every client's care plan reflects who they are, not just what they need
  • Keep us compliant and excellent — navigating state and federal home care regulations with confidence
  • Champion a culture where staff and clients alike feel they belong

What you bring:

  • Clinical nursing expertise and home care leadership experience
  • A belief that aging is something to celebrate, not just manage
  • The ability to build trust — with clients, families, and your team
  • Strong organizational skills and comfort with regulatory detail
  • A collaborative spirit and genuine care for the people you lead

If you're energized by helping older adults live fully and by building a team where everyone's unique gifts matter, we'd love to meet you.
