Job Description

Pay Range: $90,000 to $102,000

Position Summary

The DON is responsible for providing professional clinical home care services for clients.  Responsibilities include performing direct skilled nursing care, admissions, comprehensive assessments, home visits related to care or supervision, medication management and care plan updates.  Evaluates client needs for other services, making recommendations when appropriate.  Coordinates and confirms scheduling of client services. Performs scheduled and unscheduled visits as necessary and communicates plan of care with other team members.  Submits accurate and timely documentation.  Responsible for direction of care by C.N.A.’s and PCA’s and for the Management of Rhythms Home Care Direct Care Staff and quality management program. Accountable for the total recruitment life cycle, on boarding, and training of Rhythms Home Care direct care staff. Maintains a thorough knowledge of all State and Federal Regulations pertaining to home care services under State and Federal Licensing and Certification.  Coordinates with the Home Care Administrator in planning appropriate field staff with requested home care services, participates in employee reportable events and corrective actions. Participates in community outreach/marketing to include attending discharge meetings at owned communities.

Your Exciting Mission (should you choose to accept it):

  • As the primary point of contact for clients, manages and develops strong relationships with clients, family members, powers of attorney and all individuals involved in client care.
  • Conducts in-home and ongoing assessments to evaluate client service needs to assure high quality services.
  • Performs skilled home care to clients, following physician orders.
  • Partners with Human Resources to ensure that staff is at appropriate levels and recruitment needs, and for performance management as needed.  
  • Coordinates with the clinical team to ensure all appropriate assessments are conducted within the State, Local and Federal home care regulations and guidelines for all skilled and certified services. 
  • Responsible for coordinating the timely completion of all non-medical and medical home care client initial visits and routine client assessments.
  • Coordinates client care with the client physician, family/primary representative and client power of attorney.
  • Care coordination with other provider entities (i.e. Home Health, Hospitals, Communities, etc.)
  • Evaluates client nursing and medical needs and develops a plan of care per physician orders.  Continually monitors and assess client’s status
  • Responsible for setting up medicinal reminders for clients.
  • Successfully partners with all applicable parties to ensure new client admissions are complete and accurate.
  • Instructs client, family/primary representative, power of attorneys in the client plan of care and refers to community resources as necessary.
  • Revises and updates Medical, Nursing plan of care as appropriate and every 60 days by Home Care regulations and obtains physician orders and approval.
  • Responsible for the accuracy of all client charts and files.
  • Responsible for meeting and obtaining Quality Management protocols for all Medical/Skilled client care.
  • Conducts ongoing 90 day supervisory and assessment for direct care staff visits
  • Manages Home Care staff, ensures compliance with all company policies and procedures set by corporate.
  • Resolves and manages issues and concerns of clients; reports to the state as required.
  • Serves as the Management point of contact for Direct Care Staff.
  • Resolves and manages issues and concerns of clients by troubleshooting problems, resolving complaints, grievances, occurrence and reporting outcomes to the ED and submission to the state as required.
  • In coordination with the ED, conducts departmental staff meetings to include the development of agendas and the taking and distributing of minutes.
  • Responsible for accurate management of client billable hours, and other budgetary requirements as assigned. Assists in auditing financial performance.
  • Establishes, maintains all current and discharged client’s medical and non-medical files are accurate and confidential.
  • Responsible for Home Care Service Client Care Plan Updates are conducted in accordance of regulations.
  • Communicates with other community services and resources to ensure that other resources, information or education are provided to the client and/or family.
  • Maintains thorough knowledge of all State, Local and Federal Regulations pertaining to home care services provided by State Licensing and Certification.
  • Develops strategic plan for Home Care with ED.
  • Takes management on-call rotation as needed to ensure coverage.
  • Other duties as assigned

 

Qualifications

Basic Qualifications & Experience

  • Must have Colorado Registered Nursing License, in good standing. 
  • Current CPR certification.
  • Strong knowledge of governmental home health regulations.  
  • Home Care Administrator certification or able to obtain certification in the first 90 days.  
  • A minimum of 3 years’ experience in the senior health care environment.
  • Experience in a staffing or scheduling position, preferably in a Senior Living and or Home Care setting preferred.
  • Minimum 1 year experience supervising direct reports preferred.
  • Must have current Colorado driver's license in good standing. 
  • Must have excellent customer service, interpersonal, written and verbal communication skills.
  • Must be flexible and possess the ability to manage multiple priorities simultaneously.
  • Proficient in Microsoft Office applications including Excel, Word and Outlook.
  • Must be able to read, write and speak the English language.

Working Conditions / Physical Requirements

  • Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
  • Exposure to blood borne pathogens, body excretions and communicable diseases possible
  • Sits, stands, bends and moves intermittently during working hours.
  • May be subject to frequent interruptions.
  • Interacts with clients, family members and visitors frequently.
