Job Description

**Essential Duties**

  • Be the communication hub! Connect with nursing staff, physicians, pharmacists, and family members to keep everyone in the loop about resident status and celebrate wins together.
  • Champion resident and family voices—listen actively to concerns and jump into action with solutions or escalate to the right team members.
  • Attend Resident Care conferences and keep Care Plans fresh, accurate, and ready to make a real difference in residents' lives.
  • Master medication management: prepare, administer, and document medications like a pro, stay on top of ordering, and ensure expired meds get replaced promptly.
  • Document patient care issues with precision and communicate them to the team—clear communication saves lives!
  • Develop your detective skills: spot signs and symptoms early, trust your judgment, and select the best treatment approaches with confidence.
  • Lead with purpose: supervise and mentor ancillary personnel, provide constructive feedback, and foster a supportive care environment.
  • Embrace flexibility and tackle additional duties as they come your way—you're a key player on our team!

Qualifications

Basic Qualifications & Experience

  • Colorado Registered Nursing License, in good standing.  
  • Advanced training in geriatrics preferred.
  • 1-year experience in a long-term care facility preferred.
  • 1-year experience in a supervisory capacity preferred.
  • Proficient in Microsoft Office applications including Excel, Word  and Outlook
  • Must be able to read, write and speak the English language.
  • Must possess excellent customer service skills.
  • Must be able to read, write and speak the English language

Working Conditions / Physical Requirements

  • Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
  • Possible exposure to chemicals, blood borne pathogens and other infectious diseases.
  • Sits, stands, bends and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Interacts with residents, family members, visitors frequently.
