About our health programmes

Community Links drives impactful projects aimed at building healthier communities and tackling health inequalities by improving accessibility to vital healthcare services. This includes promoting early cancer diagnosis through increased participation in NHS Cancer screening programs, raising awareness, and fostering engagement across diverse health fields, such as diabetes, to ensure better health outcomes for all.

Community Links delivers targeted health programmes aimed at improving cancer screening uptake and reducing health inequalities across North East London. Work is delivered through several key projects including: the PCN Cancer Facilitator Programme and Cancer Screening Outreach. These initiatives focus on personalised, culturally sensitive engagement with underserved communities.

Community Links health team  is embarking on a new chapter, with funding secured to deliver an innovative 12-month pilot aimed at tackling health inequalities across London. As an Outreach Health Navigator, you will play a pivotal role in the Community Health Bank project, engaging with food bank users and other vulnerable communities in Newham to improve access to preventative health services and address disparities in health outcomes.

This is a frontline role where your engagement can make a real difference. You will provide advice, guidance, and support to help people overcome barriers to healthcare, while also contributing to the monitoring and evaluation of the project alongside the Project Coordinator and Programme Manager.
