Care Partners is a rapidly growing, multi-faceted healthcare organization serving Southern California. We are uniquely positioned as one of the only providers integrating Enhanced Care Management with Non-Medical Home Care, delivering a comprehensive and highly sought-after in-home care model across Orange County, Los Angeles, San Diego and the Inland Empire. The Eligibility and Authorization Coordinator serves a critical role in ensuring the accuracy, compliance, and efficiency of service eligibility and authorization processes. This individual is responsible for managing the full lifecycle of authorizations, including referral coordination, calls to referral source with updates, verifying eligibility requirements, tracking service utilization, ensuring timely renewals, and assisting to facilitate support for our offsite teams.
The ideal candidate is detail-oriented, highly organized, and passionate about supporting our Enhanced Care Management team (ECM). This role requires strong collaboration with internal & external teams, clients, to ensure seamless service delivery.
Eligibility and Authorization Coordinator - Requirements:
EDUCATION AND EXPERIENCE
- Experience in authorization coordination, eligibility verification, case management, or a related role (preferably within healthcare services).
- Strong attention to detail and exceptional organizational skills.
- Excellent interpersonal and communication skills with a compassionate, professional demeanor.
- Proficiency in Microsoft Office, EHR systems, and web-based applications.
- Bachelor’s degree preferred but not required.
- Ability to pass a federal background check.
- Valid driver’s license and proof of current auto insurance.
- Preferably Spanish or Vietnamese Speaker
Essential Functions:
- Manage the full lifecycle of authorizations, including eligibility verification, tracking utilization, and ensuring alignment with referrals and regulatory guidelines
- Maintain accurate documentation in EHR systems and ensure compliance with state, payer, and DHCS requirements, including audits
- Serve as the primary point of contact for authorization inquiries, providing clear communication and coordination with clients, families, and stakeholders
- Collaborate cross-functionally with internal teams while building strong relationships and identifying process improvements to support efficient workflows
