Job Description

The Clinical Coordinator facilitates, coordinates, integrates, and evaluates quality, safety and regulatory compliance throughout Wellness Pointe's practice to effectively improve the quality of care, ensure competencies for clinical staff and to promote a culture of safety.  This position supports organizational goals by providing quality customer service, participates in projects and supports PCMH initiatives/standards.  Leads by example and by demonstrating a commitment to team work and cooperation. 

• Ensures compliance with state, federal and local laws, regulations, and clinic policies.
• Compiles and formats data for quality assurance and improvement to address patient accident/incident reporting and investigation, infection control, along with medication monitoring and review.
• Responsible for establishing, measuring and monitoring core clinical competencies for all system nursing personnel
• Responsible for clinical education including assisting with EMR education
• Responsible for working with CMO, System Providers and Department Leaders to fully achieve all identified system clinical, quality and fiscal strategic imperatives.
• Maintains clinical expertise and competency (to include age specific competency) as well as demonstrates thorough knowledge of procedures to deliver quality primary health care.
• Documents delivery of health care and nursing process in accordance with the System’s policies and procedures in a timely, accurate, and consistent manner.
• Educates the patient and family/significant others about diagnosis and the plan of care.
• Coordinates the delivery of patient care provided by the health care team.
• Identifies and responds promptly to changes in patient’s condition and/or clinic environment.
• Coordinates the timely processing of patient referrals, prescribed medications, and other identified needs of the patient.
• Acts as a resource within the clinic to other staff members in regards to patient care.
• Administers all medications accurately using the five (5) rights of medication administration, documenting medications according to health System policy and procedures.
• Provides clinical oversight for nurses regarding clinical quality and patient safety matters through monitoring, evaluation, and coaching. 
• Participates in Compliance and Performance Improvement activities for continuous quality improvement and risk management purposes.
• Serves as a resource and positive, proactive leader.
• Accepts responsibility for remaining current with nursing and health care trends and information that impact nursing practice.
• Participates in collaboration with the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a patient's plan of care reflective of and appropriate to the assessment. 
• Participates in the evaluation of health care at the System and in monitoring health outcomes of patients.
• Is responsible for appropriate infection control and employee health programs to include training staff and monitoring compliance.
• Is responsible for the review and monitoring of clinical documentation in patient's medical records to ensure quality of care, and compliance with regulatory agencies and third-party payers.
• Travels when necessary to fulfill the entity’s needs and attends meetings as necessary to represent the System.
• Assists in the provision of technical assistance and health education in the community as requested.
• Carries out other duties as assigned.

Qualifications

• Associates degree in Nursing, (RN) required; B.S. or M.S. in Nursing preferred.
• Current unrestricted license to practice as a registered nurse (RN) in the State of Texas.
• Three (3) years of experience in a healthcare area.
• Preferred experience in an acute health care setting. 
• Mental and physical health sufficient to meet the demands of the position.
• Must be able to adhere to all client related policies and procedures including confidentiality.
• At least one (1) year leadership experience.
• Requires a valid Texas Driver’s License, appropriate driving record and auto liability
insurance that meets the minimum required by State of Texas statute.
