At Wellness Pointe, everyone will receive an exceptional quality of care. The center has expanded into a multifaceted clinic that offers obstetrics, pediatrics, general medicine, family planning, and dentistry. Some of the other services offered by our well qualified staff include education, intervention, and management of the many social issues that impact our community. Everyone is treated in complete confidence. Medicaid, Medicare, and most private health insurance is accepted.

Using an approved format to at-risk pregnant and postpartum females who have been identified as needing extra support during and after delivery. The goal of this interaction is to encourage elimination of substance usage during and after pregnancy; reinforce behavioral  change; and, provide economical support mentally and physically.

• Position will represent the program at relevant meetings throughout the community, and be familiar with community resources in order to conduct referrals and meet the needs of the targeted groups
• Will coordinate and work closely with Practice Coordinator, relevant social service and behavioral health programs of Wellness Pointe. Will also include reaching out to other relevant programs, to include faith based programs, within the region to assist in identifying appropriate eligible participants
• Will work closely with the patients in an effort to keep them engaged in the medical system during pregnancy and facilitate referral to social services and basic life needs
• Position will communicate with staff any concerns or issues needing immediate attention
• Assists senior staff in developing new program components, and other related duties
• Position requires a flexible schedule and may require working irregular hours
• Carries out other duties as assigned

• Requires minimum of Bachelors Degree in Behavioral Health field of study; or 2 years Case Management experience in Behavioral Health field; or Community Health Worker Certified
• Knowledge of available prenatal care systems and postpartum services
• Ability to conduct screenings, assessments, service plans, and follow-up with identified patients
• Requires a valid Texas Drivers License, appropriate driving record and auto liability insurance that meets the minimum required by State of Texas statue
• REliable Transportation
• Mental and physical health sufficient to meet the demands of the position
• Must be able to communicate  effectively in large group settings and demonstrate independent judgment
• Ability to understand the overall goal of the program and implement activities that meet objectives
• Must be able to keep up to date and factual documentation
• Energetic, forward-thinking and creative individual with high ethical standards and an appropriate professional image
• The ability to work independently and effectively under pressure
• Proficient in MS Office Suite
• Demonstrated human relation and effective communication skills also required
• Superior organizational and problem solving skills
