Job Description

 

  • Documents case management plans.
  • Identifies and addresses patient risk factors and/or obstacles to care
  • Identifyies patient's needs, current services, and available resources, then connecting the patient to services and resources to meet established goals.
  • Follow-up with patients and agencies as appropriate to document use/success of referral. 
  • Communicating the care preferences of the patient, serving as their advocate, and verifying the patient's needs and goals of treatment are met. 
  • Screening patients for healthcare needs.
  • Developing a patient-focused case management plan.
  • Educating the patient/family about the case management process and evaluating their understanding of the process. 
  • Provide all required documentation in a timely manner.
  • Adhere to System policy, procedures and the professional code of ethics.
  • Other duties as assigned.

Qualifications

  • Associates degree in Nursing (RN) required with 2 years experience providing psychosocial  assessments or community referrals.
  • BSN or MS preferred 
  • Knowledge of community resources and counseling/social work practices with high risk populations
  • Mental and physical health sufficient to meet the demands of the position. 
  • Must be able to adhere to all client related policies and procedures including confidentially. 
  • Requires a valid TX Driver's License, appropriate driving record and auto liability insurance that meets the minimum required by State of Texas statute.
  • Bilingual preferred.
