Job Description
We are seeking a passionate and dedicated Acute Care Physical Therapist to join our team. The ideal candidate will have a strong passion for patient care and a thirst for continued knowledge and growth.
- Assess patients to develop and initiate individualized treatment plans based on evaluation findings, patient needs, and goals
- Provide direct patient care using established protocols, evidence-based interventions, and patient care guidelines
- Document evaluation findings, treatment progress, and instructions provided to patients and caregivers accurately and timely
- Provide appropriate oversight of patient care performed by physical therapist assistants and rehabilitation technicians
- Complete a written plan of care, establish measurable goals based on evaluation findings, and develop a comprehensive treatment program to achieve those goals
- Implement individualized physical therapy treatment programs to improve mobility, function, strength, and overall independence
- Communicate effectively with patients, caregivers, and professional colleagues, including physicians, nursing staff, case management, and insurance providers, to coordinate care and support optimal patient outcomes
- Complete required documentation, including progress notes, treatment summaries, and discharge recommendations, to support continuity of care
Qualifications
- Graduate from a CAPTE-accredited Physical Therapy program
- Current state of Georgia Physical Therapy license, CPR certification
- Exercise, sports rehab, and manual-based skills
- Outgoing and energetic personality
- New graduates are welcome!
