Job Description

The Family Medicine RN Care Coordinator provides purposeful patient- and family-centered care and facilitates a partnership with the healthcare team that allows for a seamless patient experience between inpatient and outpatient settings. The Family Medicine RN Care Coordinator works to maximize the patient’s health outcomes, quality of life, and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting clinic visits, and performing outreach to patients with emergent, acute, and chronic conditions. The Family Medicine RN Care Coordinator also works to empower the patient to take ownership of and direct their own care through hands-on education, connecting to resources, and coaching on disease management and prevention. Works to develop and support a coordinated plan of care to promote smooth transitions for patients and family across the healthcare continuum that instills a sense of trust and patient advocacy.

Qualifications

  • Current MN RN license
  • BSN
  • Knowledge of Family Medicine and Primary Care
  • 3+ years of clinical nursing experience, preferably within Family Medicine or Primary Care
  • Proven ability to function at highest level within scope of practice
  • Patient education and pro-active disease management experience
  • Excellent assessment and care coordination skills
  • Experience with an EMR
  • Keyboarding/computer skills
  • Strong customer focus to accommodate patients
  • Proven ability to follow protocol and specific care delivery expectations
