The Hospice RN will provide professional and comprehensive palliative nursing services to individuals, families and groups in the community.

Utilizing the nursing process, the Home Health Registered Nurse will manage and deliver comprehensive home health services to clients with various medical/surgical diagnoses within their place of residence.  Responsible for providing nursing care to patients and education to families in their homes; coordinates activities with other community and Agency staff and carries out special activities on assignment. Provide quality care to patients and their families.  Assess, plan, and evaluate care for a complex patient population.  Respond to a full range of health and illnesses.  Adhere to the Board of Registration in Nursing rules and regulations.  

• Provides hospice-nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes.

• Collaborates effectively with other members of IDT.

• Utilizes nursing process and current standards of nursing practice in providing hospice care.

• Remains competent on technical nursing skills (i.e. IVs, Gtubes, etc.)

• Provides effective patient and family teaching.

• Understands family dynamics and works effectively within various types of family systems.

Position Type/Expected Hours of Work & Travel

This is a Full-time Day position.  Days and hours of work are based on operational needs with some flexibility based on position. 

• Active RN Licensure in MA and NH required (or willingness to obtain dual licensure)
