The Wound Care Ostomy Nurse, (WOCN), serves as a clinical resource and mentor, providing dependable consultation and guidance; provides consultation visits; direct patient care for complicated wound and ostomy patients, promotes quality care and continuous performance improvement. The WOCN provides clinical oversight of current wounds and ostomies through case review, chart audit, and home visit.  The WOCN serves as co-case manager of complex or difficult to heal wounds.  Performs home visits, staff education, clinical consultation and competency evaluation routinely, or as requested by leadership.  The WOCN carries out her/his responsibilities consistent with Agency policies, practices, mission and vision, and with instructions as set forth by the Director of Advanced Care at Home.

As a key member of the health care team, the WOCN must interact courteously and effectively with patients and their families as well as with co-workers from all Agency departments, community resources, and with patients' physicians in order to facilitate safe and efficient patient care.

Essential Functions

  • Provides oversight for patients identified as having wound or ostomy issues, consistent with the objectives of industry standards and agency wound care program.
  • Works closely with nurse managers and staff in coordinating necessary services for patients.
  • Serves as a resource regarding designated wound care program.
  • Provides dependable clinical guidance, and support to clinical managers and nursing staff.
  • Demonstrates expertise and knowledge in various areas, including, but not limited to:
    • Assures that regulatory and licensing requirements for the provision of care are met.
    • Instructs field staff on all aspects of wound care and ostomy documentation, reviews documentation for accuracy and completeness.
    • Assists in establishing plans for care delivery, including case conferencing.
    • Consults with Continuum of Care/Discharge Planning departments and other community resources including private physician’s offices.
    • Collaborates with primary nurses and other referral sources in referring patients to the wound care program.
    • Provides for professional development of the clinical and paraprofessional staff.
    • Maintains an up-to-date awareness of trends and developments in wound care and ostomy management and an awareness of Agency capabilities relative to patient needs.
    • Provides course instruction, as well as ongoing education and instruction to staff.
    • Provides for and encourages development of staff in state-of-the-art treatment techniques.  Provides ongoing monitoring.
    • Demonstrates effective teaching techniques.
    • Orients all new nursing staff to the Wound and Ostomy Care Program.
    • Consistently demonstrates the ability to establish appropriate priorities.

Position Type/Expected Hours of Work & Travel

  • This is a per diem position!

Qualifications

  • Valid MA and NH RN Licenses required, or valid license in MA/NH with expectation for licensure in the other state depending on role-per diem or regularly scheduled.
  • WOCN certification, and a minimum of 3 years WOCN experience required.
  • Minimum of 2 years of Home Health experience preferred
