Job Description

The Clinical Preceptor will mentor, develop, and coach clinical staff, which includes new graduate nurses, clinicians new to VNA and clinicians who may need further assessment and development. The Clinical Preceptor practices within the professional role, exhibiting strong clinical, case management and professional skills. This position is focused primarily on the direct in-the-field assessment, education, and supervision of novice clinicians, predominantly RNs and LPNs. 

Essential Functions

  • Orients and develops new clinical staff to their role as community health clinicians.
  • Mentors clinical staff who may need further development.
  • Collaborates with Clinical Education Coordinator to conduct regular gap analysis of staff to identify need for education e.g. OASIS, wound care, EMR and device knowledge and skill.
  • Assesses the readiness and competency of staff to function independently in their role as a community health clinician or in performing a skill.
  • Maintains a minimum weekly visit productivity of 20 RVUs/visits, which may fluctuate as agency needs are assessed by management.  Productivity visits include educational joint visits with staff.
  • Provides clinical support based on clinical practice standards and adult education principles to clinicians.
  • Provides guidance to clinicians in assessing patients and in planning appropriate interventions.
  • Provides in depth case management training in the field and pre and post visit.  Establishes a foundation of strong case management skills reflecting industry best practice including: critical thinking on patient prioritization, establishing a fluid visit pattern that reflects patient’s changing needs, commitment to continuity of care, MD communication, recognizing risk of re-hospitalization or complications, transition of care points (discharge, palliative, hospice), fundamentals of caseload review, and role as patient advocate. 
  • Instructs in accurate, timely, and appropriate clinical documentation, including use of defined templates in the EMR.
  • Educates the staff in the importance of timely synching/exporting of documentation per policy, to ensure patient safety and quality. Demonstrates and teaches critical thinking skills in the provision of patient care.
  • Provides in person in the field joint visits to evaluate clinical competency, including infusion therapy and wound care proficiency, effective use of EMR and accurate use of required documents and documentation.
  • Guides the clinician in recognition of potential patient care issues and plans collaboratively.
  • Acts as a resource to staff regarding the interpretation of the COP’s, rules, regulations, and laws relating to home health.
  • Promotes self-confidence with clinicians through constructive communication and feedback.
  • Counsels, coaches and problem-solves with clinical staff when clinical practice issues arise, as self-identified or upon request.  Communicates effectively to defined party, progress and results of interventions.
  • Meets with clinical staff on a regular basis to assess progress, and documents progress and any identified, ongoing learning needs.  Communicates to the clinical team manager and/or other clinical management staff, as appropriate.

Position Type/Expected Hours of Work & Travel

This is a full-time position, and days and hours of work are Monday through Friday, 8:00 a.m. through 4:30 p.m., with start and end times may vary depending on needs in the department.

Qualifications

  • Current RN license in the states of Massachusetts and New Hampshire (or willing to obtain dual licensure)
  • Minimum of three (3) to five (5) years clinical experience with minimum 2 years of solid home health experience.
  • BSN required
  • Strong understanding, knowledge and application of Medicare/Medicaid regulations, COPs, and standards of care/practice. Proficient in OASIS. Certification preferred.
  • Experienced in infusion therapy and wound care proficiency.
  • Minimum of two (2) years of home health as preceptor preferred.
