The Hospice and Palliative Care Clinical Coordinator is a key nursing role assisting the Clinical Team Managers with hospice clinical operations for Merrimack Valley Hospice and Care at Home palliative care to ensure optimal patient care delivery and staffing resources.  Assist with triaging patient and family calls as well as assists in the education of clinical staff in the field as needed.  Provides direct patient care revisits and admissions when deemed appropriate and necessary. Monitors the EMR workflows, ensures efficacy in clinical documentation, and works with teams and physicians to manage e-prescribing workflow/processes.

Qualifications

  • Current licensure as a RN or LPN in the Commonwealth of MA and the state of NH (or willing to obtain dual licensure)
  • Minimum three (3) years of recent clinical experience in home health or combination of acute care and home health settings. Hospice experience preferred.
