Community Health Programs for Youth (CHPY) provides a full array of primary care, counseling, and confidential services for youth and young adults between the ages of 12 – 24 through school-based clinics and in partnership with community-based organizations across San Francisco.

Job Description

Under general nursing (Lead Clinician) and medical direction (Medical Director), utilizing standardized procedures, the Physician Specialist functions within an organized system for the delivery of health care in association with other members of the multidisciplinary health care team and performs specialized provider duties including offering clinical consultation, in clinical settings of the Community Health Programs for Youth.

Location will rotate between multiple CHPY sites, including:

  • 3rd Street Youth Clinic, 1728 Bancroft Ave, SF, CA 94124
  • Balboa Teen Health Center, 1000 Cayuga Ave, SF, CA 94112
  • Cole Street Youth Clinic, 555 Cole St, SF CA 94117
  • Larkin Street Youth Clinic, 134 Golden Gate Ave, 94102

The Physician Specialist may also be assigned on an as-needed basis to another primary care site within our network to assure continuity of patient care when a provider who usually works at a site is absent or on leave.  Working alongside a multi-disciplinary care team, they would provide in-person and/or telehealth primary care and/or urgent care consistent with their medical specialty area.

Under general supervision of the CHPY Medical Director & Lead Clinician, the 2230 performs the following duties:

  • Provides routine preventative and continuity care to children and young people aged 12-25, including but not limited to well child care, urgent care, developmental screening and family planning services;
  • Orders diagnostic tests which may include laboratory, x-rays, other imaging, E.C.G.’s and other disease or condition specific diagnostic tests;
  • Identifies and manages patients with chronic diseases appropriate to clinical specialty;
  • Assesses, diagnoses, develops treatment plans, and furnishes medications;
  • Counsels, educates and advises patients;
  • Collaborates with multidisciplinary healthcare team;
  • Interfaces with community medical providers and agencies, as well as other providers in the clinic system;
  • Participates in educational and programmatic activities with the CHPY program and at assigned clinic(s), including daily huddles, weekly improvement huddles, QI projects, equity work, case conferences, and staff meetings;
  • Oversees specialty care and case management for patients ages 12-25;
  • Serves as liaison between clinic and our community partners;
  • Coordinates specialty care and case management;
  • Serves as a medical consultant to CHPY NP colleagues throughout the workweek, taking advice calls on an as needed basis and responding promptly
  • Serves a diverse population with a trauma informed and anti-racist lens including care to vulnerable populations (LGTBQ, Low socioeconomic groups, etc.)
  • Documents in the electronic medical record in a timely manner according to the SFHN standard and adheres to patient privacy and HIPPA
  • On an as-needed basis provides primary care, urgent care and relevant office based procedures to patients consistent with their medical specialty area through in-person, telehealth, and/or group visits within our broader primary care network
  • Participates in after-hours remote on-call medical advice pool

Desirable Qualifications

  • Knowledge of common Family Planning procedures including Nexplanon insertion and removal, IUD insertion and removal.
  • Facility with use of EHRs
  • Interest in working with youth including working with LGTBQI youth and working with racially diverse patients and staff
  • Two (2) years of experience in a community based or outpatient clinic providing service to the medically underserved in a multicultural environment.
  • Desire to work with low income and multicultural clients, who have multiple chronic medical and social problems.
  • Interest in community oriented primary care, managed care, quality improvement projects, teaching, and willingness to collaborate with other health professions and social service personnel.
  • Proficiency in Spanish

Qualifications

Required

  1. Possession of a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California*
    AND
  2. Successful completion** of a residency program accredited by the Accreditation Council for Graduate Medical Education or American Osteopathic Association in the appropriate medical specialty area for the assigned facility or division (i.e., Board Eligible)
    AND
  3. Possession of valid Drug Enforcement Agency registration with the United States Department of Justice***

*Applicants possessing a valid license to practice medicine issued from another state within the United States of America may apply, but if selected, the candidate will NOT be appointed/hired until they obtain a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California.

**Applicants enrolled in a residency program may apply, but if selected, the candidate will NOT be appointed/hired until they demonstrate successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education or American Osteopathic Association in the appropriate medical specialty area for the assigned facility and obtain valid Drug Enforcement Agency registration with the United States Department of Justice.

Conditions of Employment
Some positions may require valid Board Certification in medical specialty areas including, but not limited to: Family Medicine, Internal Medicine, Pediatrics, etc. If you have valid Board Certification in a medical specialty area, please respond accordingly on the Required Questionnaire.

  • Be eligible to participate in Medicare, Medicaid and/or other federal health care programs;

  • Possess a valid third-party billable provider certification (such as Medicare, Medi-Cal and/or private insurance) OR have submitted a completed billable provider application, along with the required documentation, in order to obtain the appropriate billable provider status.

The Department of Public Health (DPH) is implementing a unified Electronic Health Record (EHR) system and DPH employees must demonstrate competency in the use of the system that is appropriate for their classification as a condition of employment.  

Personal Protective Equipment (PPE)
Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e., weakens the seal between the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.
