About the Role
The Practice Success Operations Coordinator plays a pivotal role in driving operational consistency, performance management, and implementation excellence across GuideHealth’s growing network of independent physician practices.
This position provides advanced operational, analytical, and project coordination support to the Director of Practice Success and Practice Success Managers. It is a highly cross‑functional and practice‑facing role that blends workflow ownership, data management, communication support, and execution oversight.
You will serve as a trusted operations partner—ensuring our practice success initiatives are meticulously organized, data‑informed, and delivered with precision.
Core Responsibilities
Practice Success Operations & Performance Support
- Support the operationalization of practice implementation and performance management strategies.
- Assist with the ongoing monitoring of key performance indicators across value‑based care programs (HEDIS Quality, total cost of care, clinical outcomes, utilization, and engagement).
- Maintain, produce, and validate monthly practice scorecards and dashboards.
- Identify trends, performance gaps, and operational risks; escalate insights to leadership.
- Support preparation for quarterly business reviews (QBRs), performance reviews, and leadership updates.
Meeting, Planning & Leadership Support
- Coordinate and execute Practice Success governance and pillar meetings.
- Manage complex scheduling needs across leadership, internal teams, and practice partners.
- Prepare agendas, compile pre-read materials, and track follow-up actions.
- Document discussions, decisions, and next steps to ensure accountability.
Data, Reporting & Systems Management
- Maintain accurate and timely data across CRM platforms, SharePoint, and performance tracking systems.
- Compile and validate data inputs for practice reporting and internal dashboards.
- Produce recurring and ad hoc reports to support implementation, performance monitoring, and decision‑making.
- Ensure data integrity and consistency across documents, systems, and dashboards.
Practice & Stakeholder Communication
- Serve as an operational point of contact for routine practice communications.
- Coordinate distribution of newsletters, updates, surveys, and educational materials.
- Track documentation, responses, and outstanding practice deliverables.
- Triage and route practice inquiries to appropriate internal teams.
Implementation & Project Management Support
- Track onboarding milestones, timelines, and deliverables for new practice partners.
- Maintain project plans and dashboards supporting implementation and ongoing success initiatives.
- Assist in developing SOPs, workflows, and practice success playbooks.
- Contribute to process improvement efforts to enhance service delivery and operational efficiency.
Special Projects & Strategic Initiatives
- Support cross‑functional initiatives related to value‑based care implementation, engagement, and performance optimization.
- Provide analytical and operational support for pilots, new programs, and evolving practice success models.
Qualifications
Required Qualifications
Education & Experience
- Bachelor’s degree preferred (or equivalent experience).
- 4–7 years of experience in healthcare operations, project management, practice support, value‑based care, HEDIS Quality, or related performance management roles.
- Experience supporting physician practices or working within value‑based care organizations strongly preferred.
Technical Skills
- Advanced proficiency in Microsoft Excel, PowerPoint, and Outlook.
- Experience managing calendars, meetings, and complex workflows.
- Comfortable working with CRMs, reporting tools, and collaboration platforms.
- Experience with SharePoint or similar document management systems.
- Strong analytical capabilities and data organization skills.

All full-time employees of Guidehealth who work 30 hours per week or more are eligible for our comprehensive benefits package. Temporary employees and contractors are not eligible for benefits.
COMPENSATION:
The listed compensation range listed is paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.

REMOTE WORK TECHNICAL REQUIREMENTS
Guidehealth is a fully remote company. We provide new employees with the necessary equipment to function in their role at no charge to the employee. Employees provide their own internet connection, capable of conducting video calls on camera and connecting to various internal and external systems.
