  • $58.35 per hour + 12% Super + Generous Salary Packaging benefits of up to $18,550

Help people live well at home
Join our Health@Home team and play a vital role in supporting people living with chronic and complex health conditions to achieve their health goals and remain connected to their community. Through coordinated, person-centred care, you'll help individuals navigate the health system, build confidence in managing their wellbeing, and access the services they need to thrive.

Working alongside clients, carers, healthcare providers and community services, you'll be part of a collaborative team committed to improving health outcomes and reducing avoidable hospital admissions through proactive, integrated care.

Your role, your impact

  • Coordinate care for people living with chronic and complex health conditions, ensuring their goals and needs remain at the centre of planning
  • Build strong relationships with clients, carers, GPs, specialists and community providers to deliver integrated support
  • Conduct comprehensive assessments and develop tailored care plans that promote independence and self-management
  • Provide health education, coaching and practical support to improve wellbeing and health outcomes
  • Monitor client progress, identify emerging risks and respond appropriately to changing needs
  • Maintain accurate clinical documentation and contribute to continuous quality improvement initiatives

Who we're looking for

  • Current registration as a Registered Nurse with AHPRA
  • Demonstrated experience in care coordination, chronic disease management or community nursing
  • Strong clinical assessment, care planning and problem-solving skills
  • Ability to build rapport and work collaboratively with a broad range of stakeholders
  • Excellent written and verbal communication skills
  • Strong organisational skills with the ability to manage competing priorities
  • Current driver's licence

  • Experience working within multidisciplinary community health, primary care or hospital avoidance programs
  • Knowledge of chronic disease self-management models and person-centred practice
  • Experience supporting people with complex health and social needs
