Job Description
What You Get to Do:
The Community Connections DSP plays a vital role in collaborating with the Lead Skills Trainer, Area Director, residential teams, and other stakeholders to develop, implement, and evaluate Day Support Activities (DSA) services.
In this role, the DSP is responsible for delivering direct support services to individuals in accordance with their Individual Support Plan (ISP), Behavior Support Plan (BSP), Nursing Support Plan, and other applicable support documents. Staff will facilitate structured community-based activities and experiences designed to promote community participation, skill development, social engagement, relationship building, recreation, volunteerism, personal choice, and community integration.
Services may include facilitating individual and group activities, transporting individuals, coordinating community outings, supporting participation in community events, assisting individuals in achieving ISP outcomes, and documenting services provided.
The Community Connections DSP implements established Community Skills Team programming and activity schedules while promoting health, safety, dignity, respect, and self-determination.
- Enrich and support the lives of our persons served while maintaining a safe and healthy environment
- Provide companionship and personalized care while encouraging independence and self-care.
- Assist with activities of daily living, including toileting and eating.
- Provide mobility and transfer assistance, which may include using mechanical lift devices.
- Implement positive behavior supports and de-escalate challenging behaviors when needed.
- Other duties as assigned

Qualifications
What Makes You a Great Fit:
- 1+ years of experience working with individuals with intellectual and developmental disabilities, mental health diagnoses, aging populations, recreation services, education, healthcare, or other human services fields is required.
- Valid driver's license and vehicle with auto insurance is required.
- Must be at least 18 years old
- Must be able to provide references
- Able to attend two weeks of PAID in-house orientation and training
- Have a passion to help others
- People-oriented and have a team approach
- Dependable, adaptable, flexible
- Good communication is key in this role. You’ll rely on your reading, writing, and communication skills to document care, follow support plans, and collaborate with both internal and external team members.
