Nurse Practitioner - Triage

  • Location: San Bernardino, CA
  • Compensation: $150,000 - $170,000 *Negotiabe 
  • Schedule: Monday – Friday, 8:00 AM – 5:00 PM
  • NO Nights, NO weekends, or Holidays! IN CLINIC, NO HOME VISITS
  • Light patient volume (8-10 patients/day)

Requirements - Nurse Practitioner - Triage: 

  • Current CA State issued Nurse Practitioner license 
  • Current active DEA license
  • 1+ year as a practicing Nurse Practitioner 
  • 5+ years a Registered Nurse 
  • Experience in elderly care/geriatric setting

Overview of Responsibilities:

The Nurse Practitioner - Triage Team provides advanced clinical support by evaluating urgent participant concerns, managing acute conditions, and coordinating care. This role ensures timely intervention and serves as a clinical leader for triage operations. 

  • Serve as the primary clinical resource for urgent participant needs.
  • Perform rapid assessments, physical exams, and order/interpret diagnostic tests for triage cases. 
  • Formulate treatment plans, prescribe medications, and provide immediate interventions as needed. 
  • Manage participant and staff inquiries, prioritizing care and routing appropriately. 
  • Educate participants, caregivers, and staff on urgent care protocols and health management. 
  • Collaborate with interdisciplinary team members to ensure continuity of care. 
  • Document all triage encounters and interventions in compliance.
  • Provide leadership in managing medical emergencies within the center. 
  • Other duties as assigned.
