Job Description

We are seeking an Outreach and Admissions Coordinator to engage, educate, and expand our referral of hospital, private concierge and hospice network, outpatient clinics, assisted living facilities, skilled nursing facilities, and community organizations. This person will play a central role in coordinating admissions to the Guest House (GH) - a nationally recognized 6-bed residential care facility that offers a unique model of compassionate 24-hour care, seven days a week for those facing advanced illness at the end of life.

 

As our Outreach and Admissions Coordinator you will:

• Drive the outreach, development and management of community partnerships

• Collaborate with the Director of Guest House Caregiving Services and Nurse Manager to maintain positive working relationships with current referring organizations and develop collaborative agreements with new organizations

• Assess and perform site visits by reviewing available clinical information and eligibility for admission 

• Maintain the list of patients who are prospective GH residents

• Support the psychosocial needs of residents, families, and caregivers for ongoing comfort and care

• Engage with residents, families and caregivers to help ensure their spiritual, clinical and emotional well-being needs are addressed

Qualifications

Our ideal candidate will possess the following qualifications.

  • Bachelor's degree in a related field
  • 2 years of outreach and/or admissions experience (highly desired)
  • 3 years of experience working in end-of-life care including 1 year in a residential care facility (highly desired)
  • Ability to work in an intimate, emotionally charged setting
  • Excellent interpersonal and written communication skills
