HEALTH ASSESSOR
Vital Research is a national research consulting firm, specializing in aging and disabilities research. We are looking for Health Assessors throughout Minnesota to complete in—person health assessments with persons who have a physical, developmental, or mental health disability. Health Assessors must be registered nurses, social workers, and/or case managers.
POSITION DESCRIPTION
• Independent contractor
• Up to 12 weeks between beginning of October, 2016 and December 31st, 2016
• All-inclusive rate per completed interview of $100-$150 based on driving distance
• Schedule and conduct a minimum of 10 face-to-face interviews per week
• Print one-page assignment information for up to 100 potential respondents
• Travel to location of respondent’s choice, usually their home
• Keep organized records of schedules/assessments for weekly reports
• Conduct health assessments lasting 30-60 minutes using a provided handheld device
• Upload completed assessments daily using a WiFi connection
REQUIREMENTS
• Must have a minimum of a Bachelor’s degree in Nursing, Behavioral Science, Social Work or related field
• Must have professional training and experience in conducting health assessments
• Participate in a mandatory 1 day, paid training session, half of which will be conducted in person in St. Paul in early October 2016. The other half is to be done at home through webinars and web-led exercises
(stipend to offset travel/lodging will be provided for those more than 1.5 hours from St. Paul)
• Interaction w/the disabled, older adults, and those with mental health illness
• Strict compliance w/confidentiality, data security, and gov’t. HIPAA regulations
• Read and record responses accurately on a provided handheld tablet/laptop device
• Skilled communicator in person, in writing (e.g. email), and by phone
• Proficient with email, uploading/downloading files, and Internet use
• Must have a functioning personal computer, printer and reliable access to wired and wireless Internet, and adhere to all technology and data security requirements, including current anti-virus and malware protection software and performance of data security checks, as needed
• Own reliable transportation, have valid driver’s license/automobile insurance
• Willing and able to drive 50 miles/1 hours to complete assessments
• Pass training requirements, criminal background and driving record check
• Bilingual is a plus, in particular Somali, Hmong, Spanish, Russian, and Vietnamese
